On the first day of the calendar month following sixty
consecutive days of employment, a full time employee may be
eligible for group medical, dental, vision, life and long-term
disability insurance coverage under the Company-sponsored group
plans. Dependents of full-time employees may be eligible for
group medical, dental and vision insurance coverage under the
Company-sponsored group plans. Employees should consult the
Summary Plan Description for more complete information about
eligibility and the details of the Company-sponsored group
insurance plans. Copies of the Plan Documents and Summary Plan
Descriptions are available from the Corporate Human Resources
Director. Temporary and part-time employees are not eligible for
these benefits. The Plan Documents are controlling. Currently,
the Company pays a portion of the premium for group medical,
dental, vision, life and long-term disability insurance coverage
for an employee. The employee must pay the balance of the
premium, and the entire premium for dependent insurance
coverage. |
